The City of Toledo and Dental Center of NW Ohio entered into agreements for US EPA Revolving Loan Fund Program subgrant for $42,000 for asbestos abatement and universal waste disposal at 2116 Madison Ave. July 19, 2019: The loan agreement were terminated by all parties due to the Dental Center experienced a cyber security breach of their computer system in late 2018 which expended their capital funds which were budgeted for the demolition of 2116 Madison Ave. The City also received notification that the capital funds raising campaign for the construction of the new facility (which would be constructed on the former building's footprint) was unsuccessful so abatement activities for the remaining buildings on this property could not proceed.

Former Use: The site is 1.06 acres in area and consists of ten parcels. The site includes paved parking areas, grassy areas with trees, a small foundation, three buildings (2116, 2130, and 2138 Madison Avenue), and a garage. The site is located on the north side of Madison Avenue between 21st Street and 22nd Street. The site was residential in at least 1895 with three residences located on the western portion of the site and two residences located on the eastern portion of the site. Between 1895 and 1905, two apartment buildings were constructed on the eastern portion of the site. During the 1930s through the 1950s, some of the residences were used as offices or nursing/assisted living facilities. Between 1954 and 1957, the residences/offices on the western portion of the site were demolished and the current three office buildings were constructed. The offices were occupied by accountants, insurance agencies, engineers, art studios, sales, and a dentist. The two apartment buildings located on the eastern portion of the site were demolished between 1985 and 1987. The 2138 Madison Avenue building and second floor of the 2130 Madison Avenue building are occupied by dental offices. The remaining portions of the 2130 Madison Avenue building and the 2116 Madison Avenue building are unoccupied.
